Is the establishing shot such a big deal?

Well, it sort of is, especially if a 180 degree shot comes after it. It's also one of the most efficient compositions, and it can minimize the amount of shots per scene. However, it does this at the expense of intimacy and detail. But within the context of, say, hitting a pedestrian in the leg with your car, it can make a shot look remarkably clear.
